How to Sign Up
Welcome to Mermaid Transformations! To ensure a smooth and enjoyable experience, all participants are required to complete the following forms and prerequisites before attending our sessions: RSVP to you intended programs event. Complete your booking on Splash Water Sports Website. (We can provide this link) Complete PADI Online Mermaid E-Learning: This online course provides essential knowledge and safety information about mermaiding. It is a required step to ensure all participants are well-prepared and informed. Swimming and Floating Competency: Participants should feel comfortable swimming and floating on their backs. Basic swimming skills are essential for safety and to fully enjoy the mermaid experience. > Liability Release/Assumption of Risk/Non-Agency Acknowledgement (We will send you this form) > Diver Medical Form (10346) (we will send you this form) A medical evaluation to ensure participants are fit for underwater activities. This form must be filled out honestly and completely before the start of class. Due to the nature of our classes, there are no refunds. However, we will allow you to reschedule with advanced notice. To reschedule, please provide 24 hours advanced notice.
